A.C. Read requires that EVERYONE over the age of 16 who is playing in an on-base event must complete and sign the form attached (unless that person holds a military ID). You must also have your ID with you. If we do not receive your form, you will be on your own to get a pass.
Passes can be obtained in advance at the gate. Our Sponsor is AC Read Golf Course and the form must be SIGNED. We do not need a copy of your DL, you will show that at the security gate.
