I hereby authorize First Tee Gulf Coast (GPJGA), its agents and assigns and all other media outlets to use and reproduce my name, photograph(s), and/or videotape recordings of me or audio recordings of my voice for any and all purposes including promotion and advertising. I acknowledge that First Tee Gulf Coast will be the sole owner of all the rights in and the recording thereof, for all purposes. First Tee Gulf Coast shall have the right to telecast the Tournament, or the recording thereof, one or more times on a sustaining or commercial basis, over any cable or network television system as well as any affiliate system or stations. I agree to hold you and any third parties harmless against any liability, loss or damage (including reasonable attorney's fees) caused by or arising from the telecast of my appearance, or any utterance made by me or material furnished by me in connection with my participation therein.

Please initial above: I, the parent/legal guardian of the above named youth, give approval for participation in First Tee Gulf Coast sponsored activities. I understand that FTGC is an independent organization which benefits from the participation of many golf organizations including The First Tee home office and its oversight organizations PGA TOUR, PGA of America, USGA, Augusta National Golf Club, and the LPGA. I assume all risks of injury whatsoever and agree to hold harmless FTGC and The First Tee home office including its oversight organizations from claim(s) of any nature arising from any activity, including transportation, connected with The First Tee Chapter facility or program(s). This hold harmless agreement includes, but is not limited to, any claim due to injury resulting from negligence of The First Tee Chapter, The First Tee home office and its oversight organizations, employees, agents, LPGA or PGA Professionals, participating agencies, and volunteers. I consent to The First Tee Chapter and The First Tee home office communicating information regarding my child’s participation via the internet.
